When our database loads it reports 12 disk fragments. Is this referring to
table fragmentation? If so, how do I correct it? I've read about
sa_table_fragmentati
on() but I am not sure how to interpret the results. Can
someone point me in the right direction?

This is referring to the file fragmentation of the database file. Please see
the following documentation for instructions on how to correct the file
fragmentation:
